ElderLaw 101 Extending Medicare to the 55-64 age group—who have relatively high health care costs—is a potential fix that could insure the near-elderly and provide stability to the marketplaces. It would remove expensive individuals and families from coverage by private insurance companies, who could in turn reduce premiums for individuals and families below the age of 55.

The individual fee for each Part C beneficiary is also uplifted or downsized slightly (approximately 1%-3% in either direction on average) from the county-specific fee based on a risk-based formula tied to the personal health characteristics of the capitated individual. The theory is that the risk-based formula will not affect spending, but in practice it almost always increases cost per beneficiary by one or two percent, either because the Medicare Advantage plan is diligent in upcoding to a beneficiary’s specific risks or because patients on Original FFS Medicare, where providers have no incentive to code at all, are undercoded .

Medicare Advantage plans, also known as Medicare Part C, are one way for beneficiaries to receive their Medicare benefits. These plans are required to offer everything that’s covered under Original Medicare, Part A and Part B, with the exception of hospice care, and may include other benefits as well, such as prescription drug coverage, dental, and vision.

People joining Medicare for the first time have an Initial Enrollment Period, during which they can also select a Medicare Advantage plan. People can also join, switch, or disenroll from a Medicare Advantage plan during the annual Open Enrollment Period from Oct. 15 to Dec. 7. There are also Special Enrollment Periods for certain circumstances, such as when a senior moves from one plan’s service area to a new location.

Three months before you turn 65, or when you want to start coverage, contact Medicare to enroll in Medicare Parts A and B, so that you’ll get coverage beginning the month you actually reach your 65th birthday or retirement after age 65.

  3. Medicare Part C plans must cover all the same things as Medicare Part A and B. They also may cover services that Original Medicare does not pay for. Examples are eye exams, a pair of eyeglasses each year or a hearing exam. They may charge different amounts than you would pay through Original Medicare.

If you live in Puerto Rico, and are already receiving benefits from Social Security or the RRB, you will automatically be enrolled in Part A, but not Part B. Anyone in this situation needs to complete an application to enroll in Part B.

People living in the United States and U.S. Territories (except Puerto Rico) who are already collecting Social Security—either disability or retirement—are automatically enrolled into Part A and Part B when they’re first eligible. These people will get a packet of information a few months before they turn 65 or receive their 25th month of Social Security Disability or Railroad Retirement Board (RRB) benefits.  At that time, they can choose to keep or decline Part B, but can’t decline Part A unless they withdraw their original application for Social Security and pay back all Social Security cash benefits.

Coverage start dates vary based on when you sign up. If you sign up during the first three months of your IEP (i.e., before your birthday month), then your health insurance will take effect on the first day of your birthday month. If your birthday falls on the first of the month already, then your coverage will start on the first of the previous month. The longer you wait to sign up, the later your coverage will start, which is why it’s a good idea to check your eligibility and enroll as soon as you’re able.
If you’re terminated from Medicare Part B and no longer paying Part B premiums to the SSA, you must contact CalPERS immediately regarding your termination. If you receive reimbursement for Part B premiums on your retirement warrant for months that you did not pay Part B premiums to the SSA, CalPERS will invoice you for the overpayment of Medicare Part B reimbursements.

Medicare has four basic parts – A, B, C and D. If you’re unfamiliar with how they work, read Medicare 101: Do You Need All 4 Parts? Taken together, Parts A (hospital care), B (doctors, medical procedures, equipment) and D (prescription drugs) provide basic coverage for Americans 65 and older. What’s relevant for this article is what these parts don’t cover – deductibles, co-pays and other medical expenses that could wipe out your savings should you become seriously ill. That’s where Part C comes in. Also known as Medicare Advantage, it’s one of two ways to protect against the potential high cost of an accident or illness. Here’s what could happen.

When you apply for Medicare, you can sign up for Part A (Hospital Insurance) and Part B (Medical Insurance). Because you must pay a premium for Part B coverage, you can turn it down. However, if you decide to enroll in Part B later on, you may have to pay a late enrollment penalty for as long as you have Part B coverage. Your monthly premium will go up 10 percent for each 12-month period you were eligible for Part B, but didn’t sign up for it, unless you qualify for a special enrollment period.
