Diane – R.I.: Do all drug manufacturers sell their drugs to Medicare Part D plans at the same price, or do Part D plans negotiate drug prices with manufacturers? In other words, is it possible to pay less for what is generally considered a Tier 3 drug (very expensive) by shopping around for a Part D plan? My script generally increases in price by more than $2,000 every three months. My most recent script for a three-month supply cost my Medicare Part D insurer $20,000. Thank you.

Inpatient Rehabilitation Facility PPS To ensure that Medicaid beneficiaries considered for default enrollment upon their conversion to Medicare are aware of the default MA enrollment and of the changes to their Medicare and Medicaid coverage, we also propose, at § 422.66(c)(2)(i)(C) and (c)(2)(iv), that the MA organization must issue a notice no fewer than 60 days before the default enrollment effective date to the enrollee. The proposed revised notice must include clear information on the D-SNP, as well as instructions to the individual on how to opt out (or decline) the default enrollment and how to enroll in Original Medicare or a different MA plan. This notice requirement aims to help ensure a smooth transition of eligible individuals into the D-SNP for those who choose not to opt out. All MA organizations currently approved to conduct seamless conversion enrollment issue at least one notice 60 days prior to the MA enrollment effective date, so our proposal would not result in any additional burden to these MA organizations using this process. Recent discussions with MA organizations currently conducting seamless conversion enrollment have revealed that several of them already include in their process additional outreach, including reminder notices and outbound telephone calls to aid in the transition. We believe that these additional outreach efforts are helpful and we would encourage their use under our proposal.

Your monthly costs will depend, of course, on the precise drugs you and your wife need to take. There also could be what I call a convenience factor at work here. More and more drug plans are doing preferential deals with big drugstore chains. The insurer and, to a lesser extent, you, get better drug prices and the chain gets preferred access to consumers. Drug plans with these deals may charge higher prices if you get your prescriptions filled at a pharmacy that’s not part of its preferred network. Your favorite neighborhood pharmacy could be the odd man out here. You need to consider if that’s OK or if you’re willing to pay extra for convenience and to keep hearing your pharmacist laugh at your stale old jokes.

The Patient Protection and Affordable Care Act (“PPACA”) of 2010 made a number of changes to the Medicare program. Several provisions of the law were designed to reduce the cost of Medicare. The most substantial provisions slowed the growth rate of payments to hospitals and skilled nursing facilities under Parts A of Medicare, through a variety of methods (e.g., arbitrary percentage cuts, penalties for readmissions).
(4) Additional Considerations Call Unless you have retiree health insurance, you’ll probably want a medigap policy to help cover co-payments and deductibles, and a Part D drug plan to cover prescription drugs. Part D averages $32 per person (plus a high-income surcharge that boosts premiums by $12.30 to $70.80 per person if income is above $85,000 for singles or $170,000 for couples). The most popular medigap policy, Plan F, has a median premium of $172 per month, according to Weiss Ratings.

Under a point-of-sale rebate policy designed as we have described in this comment solicitation, beneficiaries would see lower prices at the pharmacy point-of-sale, and on Plan Finder, beginning immediately in the year the policy takes effect. Lower point-of-sale prices would result directly in lower cost-sharing costs for non-low income beneficiaries, especially for those who use drugs in highly competitive, highly-rebated categories or classes. For low income beneficiaries whose out-of-pocket costs are subsidized through Medicare’s low-income cost-sharing subsidy, cost-sharing savings resulting from lower point-of-sale prices would accrue to the government. Plan premiums would likely increase as a result of such a point-of-sale rebate policy—if some rebates are required to be passed through to beneficiaries at the point of sale, fewer such concessions could be apportioned to reduce plan liability, which would have the effect of Start Printed Page 56425increasing the cost of coverage under the plan. At the same time, the reduction in cost-sharing obligations for the average beneficiary would likely be large enough to lower their overall out-of-pocket costs. The increasing cost of coverage under Part D plans as a result of rebates being applied at the point of sale likely would have a more significant impact on government costs, which would increase overall due to the significant growth in Medicare’s direct subsidies of plan premiums and low income premium subsidies.

By JEREMY WHITE The cost of coverage would be offset significantly by reducing health care costs. The payment rates for medical providers would reference current Medicare rates—and importantly, employer plans would be able to take advantage of these savings. Medicare Extra would negotiate prescription drug prices by giving preference to drugs whose prices reflect value and innovation. Medicare Extra would also implement long overdue reforms to the payment and delivery system and take advantage of Medicare’s administrative efficiencies. In this report, CAP also outlines a package of tax revenue options to finance the remaining cost.

Just a bit of Medicare Maven humor given the skyrocketing prices of many generics. Hey, I feel your pain — literally. I also get to pay an outrageous amount of money so I can stick a spring-loaded injector into my body. But enough of such fun. Part D plans are able to negotiate drug prices with manufacturers. That means drug prices can vary by plan. However, it’s unusual for them to jump around a lot during a plan year. So, you might ask your insurer what’s up with that.
