If you think the government is favoring MA insurers, you are correct. The key reason is that MA plans are based on managed medical care that can tie services to health outcomes. Original Medicare has been a fee-for-service program that is not structured to measure such outcomes, although it has experimented with accountable care programs that could be expanded to furnish such benefits and performance metrics. A 2001 study by the Government Accountability Office evaluated the quality of responses given by Medicare contractor customer service representatives to provider (physician) questions. The evaluators assembled a list of questions, which they asked during a random sampling of calls to Medicare contractors. The rate of complete, accurate information provided by Medicare customer service representatives was 15%.[100] Since then, steps have been taken to improve the quality of customer service given by Medicare contractors, specifically the 1-800-MEDICARE contractor. As a result, 1-800-MEDICARE customer service representatives (CSR) have seen an increase in training, quality assurance monitoring has significantly increased, and a customer satisfaction survey is offered to random callers.

Medicare prescription drug coverage (Part D)
[[state-end]] Chemung Our estimate for the amount of time that MAOs and Part D sponsors would spend on administrative tasks related to the MLR reporting requirements under this proposed rule is based on our current burden estimates that are approved by OMB under control number 0938-1232 (CMS-10476), where we estimated that, on average, MA organizations and Part D sponsors would spend approximately 47 hours per contract on administrative work related to Medicare MLR reporting, including: Collecting data, populating the MLR reporting forms, conducting a final internal review, submitting the reports to the Secretary, and conducting internal audits. Inadvertently, our currently approved estimate did not specify (or break out) the portion of the overall reporting burden that could be attributed solely to the tasks of preparing and submitting the MLR report. We are correcting that oversight by estimating that the burden for preparing and submitting the MLR report is approximately 11.5 hours (or 24.4 percent of the estimated 47 total hours spent on all administrative work related to the MLR reporting requirements) per contact.

However, we also believe that MA organizations’ and Part D sponsors’ fraud reduction activities can potentially provide significant value to the government and taxpayers by reducing trust fund expenditures. When MA organizations and Part D sponsors prevent fraud and recover amounts paid for fraudulent claims, this lowers the overall cost of providing coverage to MA and Part D enrollees. Because MA organizations’ and Part D sponsors’ monthly payments are based in part on their claims experience in prior years, if MA organizations and Part D sponsors pay fewer fraudulent claims, this should be reflected in their subsequent cost projections, which would ultimately result in lower payments to MA organizations and Part D sponsors out of the Medicare trust funds, and could also result in lower premiums or additional supplemental benefits for beneficiaries.

Therefore, even though Medicare Parts A and B provide some coverage for your potential health care and hospitalization needs, this program also leaves a number of “gaps” in terms of out-of-pocket costs for enrollees. In fact, although the coverage provided by the government is extremely affordable and beneficial, it tends to cover only 80% of your medical expenses. In many cases these uncovered charges (e.g. deductibles, copays, prescriptions and other expenses) can really add up – potentially even making some services cost prohibitive or causing financial hardship.

The FreeStyle Libre works in a slightly different way. A sensor, worn on the upper arm for up to 10 days, collects glucose data from just below the skin. Users move a reader device, which looks like a large glucose meter, over the sensor to check their glucose level. Like traditional CGMs, the FreeStyle Libre displays whether a person’s glucose is rising, falling, or staying steady, but the device doesn’t deliver alerts when levels go out of range. On the upside, the FreeStyle Libre doesn’t require calibrations using finger-stick values, whereas other CGMs currently do.
