CMS annually releases policy and payment updates to the MA program through a “Medicare Advantage and Part D Rate Announcement and Call Letter.” In April 2018, CMS finalized its CY19 Call Letter which included 1) a reinterpretation of “primarily health-related” supplemental benefits, and 2) a reinterpretation of the “uniformity requirement” for MA plans. Medicare pays for a clinical breast exam (CBE) once every 24 months for women at average risk of breast cancer. A CBE is covered once every 12 months for women at high risk and those of child-bearing age who have had an exam that showed cancer or other changes in the past 3 years. (The CBE is usually done at the same time as your pelvic exam. See the “Cervical cancer” section below.) You pay nothing for these exams if the doctor accepts assignment.

That means that every U.S. citizen can enroll in Medicare starting at age 65 (or earlier based on disability, which we’ll discuss below). When we say “Medicare,” we’re referring to original Medicare. This comprises Parts A and B. Part A covers hospital care while Part B covers medical care. There are four parts to the program (A, B, C and D); Part C is a private portion known as Medicare Advantage, and Part D is drug coverage.
