Read Our Stories Kaiser Health News As you’ve seen in the chart, the large majority of Medicare Cost Plan enrollees are in Minnesota. Because the Minnesota Medicare landscape has been dominated by Cost Plans, the market is ripe for carriers to offer alternative options, such as Medicare Advantage and Medicare Supplement plans. For instance, Blue Cross and Blue Shield of Minnesota (BCBSMN), which traditionally sold Medicare Cost Plans prior to the 2018 plan year, now has two Medicare Advantage plans available in 55 counties. And Minneapolis-based Medica has expanded its portfolio with a new Medicare Supplement plan for Minnesota residents as of March 2018. Other major national carriers, including Aetna and UnitedHealthcare, are planning to expand in the Minnesota market in 2018 for the 2019 AEP.

Medicare is not generally an unearned entitlement. Entitlement is most commonly based on a record of contributions to the Medicare fund. As such it is a form of social insurance making it feasible for people to pay for insurance for sickness in old age when they are young and able to work and be assured of getting back benefits when they are older and no longer working. Some people will pay in more than they receive back and others will receive more benefits than they paid in. Unlike private insurance where some amount must be paid to attain coverage, all eligible persons can receive coverage regardless of how much or if they had ever paid in.
Other changes in benefit packages could be made based on market competition or other considerations, putting upward or downward pressure on premiums, depending on the particular change. Changes would be expected to be minimal as long as the current essential health benefits (EHB) requirement is in place. Other plan design features, such as drug formularies and care management protocols, also could affect premium changes.

The purpose of the current policy is to provide Part D plan sponsors with specific guidance about compliance with § 423.153(b)(2) as to opioid overutilization, which requires a Part D plan sponsor to have a reasonable and appropriate drug utilization management program that maintains policies and systems to assist in preventing overutilization of prescribed medications. We adopted the current policy on January 1, 2013, and it has evolved over time in scope in several ways with stakeholder feedback and support, including through the addition of the OMS in July 2013, primarily via the annual Parts C&D Call Letter process.

The increase in costs of medical services and prescription drugs—referred to as medical trend—is based on not only the increase in per-unit costs of services, but also changes in health care utilization and changes in the mix of services. Projected medical trend in 2018 is expected to be consistent with 2017 medical trend; estimates are in the 5 percent to 8 percent range.1 The growth in spending for prescription drugs has leveled off somewhat, as many relatively new high-cost drugs (e.g., those treating hepatitis C) are now built into the base. As a result, spending for prescription drugs is expected to only slightly outpace the costs for other medical services.

Any day now, the Trump administration is expected to release new regulations to make short-term health-insurance plans last a lot longer. In a fact sheet about the forthcoming changes, the administration said it wants to extend access to the plans—which now expire after three months, and offer too few services to qualify for the Affordable Care Act’s tax credits—in order to “provide additional, often much more affordable coverage options, while also ensuring consumers understand the coverage they purchase.” According to that release, the policies are beneficial for unemployed people and for those who can’t afford pricey Obamacare plans. But are they?

Most people should enroll in Part A when they turn 65, even if they have health insurance from an employer. This is because most people paid Medicare taxes while they worked so they don't pay a monthly premium for Part A. Certain people may choose to delay Part B. In most cases, it depends on the type of health coverage you may have. Everyone pays a monthly premium for Part B. The premium varies depending on your income and when you enroll in Part B. Most people will pay the standard premium amount of $134 in 2018.
