For example, Medigap policies sold after January 1, 2006, no longer include prescription drug coverage, but if you purchased your plan before then, you can keep the older policy. You may want to hang on to your older Medigap policy if it includes coverage for prescription drug expenses, and changing Medigap plans would dramatically increase your out-of-pocket costs for prescription drugs.

Most people should enroll in Part A when they turn 65, even if they have health insurance from an employer. This is because most people paid Medicare taxes while they worked so they don't pay a monthly premium for Part A. Certain people may choose to delay Part B. In most cases, it depends on the type of health coverage you may have. Everyone pays a monthly premium for Part B. The premium varies depending on your income and when you enroll in Part B. Most people will pay the standard premium amount of $134 in 2018.

It is with these concerns in mind that we are proposing to reduce the current reporting burden to require the minimum amount of information needed for MLR reporting by organizations with contracts to offer Medicare benefits. Specifically, we are proposing that the Medicare MLR reporting requirements would be limited to the following data fields, as shown in Table 12: Organization name, contract number, adjusted MLR (which would be populated as “Not Applicable” or “N/A” for non-credible contracts as determined in accordance with §§ 422.2440(d) and 423.2440(d)), and remittance amount. We solicit comment on these proposed changes.

(TTY: 711) Many look to the Veterans Health Administration as a model of lower cost prescription drug coverage. Since the VHA provides healthcare directly, it maintains its own formulary and negotiates prices with manufacturers. Studies show that the VHA pays dramatically less for drugs than the PDP plans Medicare Part D subsidizes. One analysis found that adopting a formulary similar to the VHA's would save Medicare $14 billion a year (over 10 years the savings would be around $140 billion).
At the same time, you can also enroll in Medicare Part B, which covers doctors' visits and outpatient care. This coverage exacts a monthly premium ($104.90 for most people in 2013), plus a deductible and coinsurance. (If you're collecting Social Security when you turn 65, you will automatically be enrolled in Part A and Part B, and the Part B premium will be deducted from your benefits.) If you still have health coverage through work or are covered by your spouse's employer, you may be better off keeping that coverage and delaying Part B.
