When the FEHB plan is the primary payer, the FEHB plan will process the claim first. If you enroll in Medicare Part D and we are the secondary payer, we will review claims for your prescription drug costs that are not covered by Medicare Part D and consider them for payment under the FEHB plan.
It gets more complicated from there. Let’s say Phoenix Man has his hit-by-a-bus moment and suffers a serious, but not deadly, injury like a complex and displaced arm fracture. Assuming he doesn’t have the wherewithal or pain tolerance to take a Lyft to the hospital, and decides to take an ambulance, the ride might set him back $1,000. If this is his first health incident since enrolling in the plan, that payment would come straight from his own checkbook, because his deductible hasn’t been met. While it only allows for some very rough assumptions, health-cost calculator site Amino says Phoenix Man can expect another $5,000 in facility fees. The costs of the actual medical procedure to fix his arm would be about $4,000, of which he’d pay half, since by then his coinsurance payments would kick in. Assuming things go well and there aren’t complications, Phoenix Man would pay around $7,500 for a $10,000 treatment.
Reward factor means a rating-specific factor added to the contract's summary or overall ratings (or both) if a contract has both high and stable relative performance.Start Printed Page 56497

Of the Medicare beneficiaries who are not dual eligible for both Medicare (around 20%) and Medicaid or that do not receive supplemental insurance via a former employer (40%) or a public Part C Medicare Advantage health plan (about 30%), almost all elect to purchase a type of private supplemental insurance coverage, called a Medigap plan (20%), to help fill in the financial holes in Original Medicare (Part A and B). Note that the percentages add up to over 100% because many beneficiaries have more than one type of supplement. These Medigap insurance policies are standardized by CMS, but are sold and administered by private companies. Some Medigap policies sold before 2006 may include coverage for prescription drugs. Medigap policies sold after the introduction of Medicare Part D on January 1, 2006 are prohibited from covering drugs. Medicare regulations prohibit a Medicare beneficiary from being sold both a public Part C Medicare Advantage health plan and a private Medigap Policy. As with public Part C health plans, private Medigap policies are only available to beneficiaries who are already signed up for benefits from Original Medicare Part A and Part B. These policies are regulated by state insurance departments rather than the federal government though CMS outlines what the various Medigap plans must cover at a minimum. Therefore, the types and prices of Medigap policies vary widely from state to state and the degree of underwriting, open enrollment and guaranteed issue also varies widely from state to state.

You don’t pay a premium for Medicare Part A, which covers hospitalization. But for Medicare Part B, which covers outpatient care, most people pay $104.90 per month. Single enrollees earning more than $85,000 and married enrollees filing jointly and earning more than $170,000 pay $146.90 to $335.70 per person per month.
Specialty tier means a formulary cost-sharing tier dedicated to very high cost Part D drugs and biological products that exceed a cost threshold established by the Secretary.

The total number of cases in Equation 3 is the number of cases that should have been in the IRE for the Part C TMP data.
